I do hereby release Oregon Moo Duk Kwan (OMDK)  and all others associated with producing this event in any capacity for any liability due to injuries, etc., that I may incur as a result of my attendance or participation. I clearly understand that participation in the sparring aspect of this event could involve bodily contact. I am fully aware of my personal medical condition and hereby certify that I am mentally and physically fit to participate in the event(s) for which I have registered and that I have the medical coverage or personal means to cover the expenses. I permanently waive any compensation for photos, videos, media coverage, etc. of me that may be utilized by Oregon Moo Duk Kwan.
